Dr. Giggles

This horror features Evan Rendell, a psychopath whose father killed off a bunch of the local townsfolk. When Evan escapes from the mental institution he was imprisoned in, he travels back to his home town, posing as a true doctor, and seeks revenge on the citizens of the town where his father was captured.
